I have used the Modular Docbook DSSSL Stylesheets for the formatting of this article. I have had to change two things in the stylesheets:
Comment to Remark in dbblock.dsl and dbfootn.dsl in order to support DocBook 4.0.
"Chapter %n" to "Chapter %n, %t" in dbl1en.dsl.
Apart from this, I have made some minor customizations to adjust the presentation, but these do not affect the stylesheets themselves.
